Ø Must be a senior graduating from Fremont High School; or from a parochial school within the Fremont School District and be a resident of the Fremont School District.

Ø Must provide a copy of your high school transcript including your ACT score.

Ø Must provide a copy of high school list of documented community service hours.

Rotary is an international service club with more than 1.2 million members worldwide. The mission of Rotary is to provide service to others, promote high ethical standards and advance world understanding, goodwill, and peace through its fellowship of business, professional, and community leaders. The motto of the group is “Service Above Self”.

Scholarship winners are selected based upon exemplary citizenship and character expressed through community activity, personal and academic achievement, and service above self. Service above self can be described as service to your community or others for the pure joy of providing assistance. Service above self is not providing service simply because you are a member of a group or have a requirement to complete community service.

Based on the above, please provide the following information (use a separate piece of paper):

1. Describe how you have demonstrated exemplary character in past (paid or unpaid) work experiences.
[Exemplary Character is demonstrated through trustworthiness (honesty/reliability/loyalty), respect (Golden Rule/tolerance), responsibility (self-discipline/accountable), fairness, and caring (kind/compassionate/helpful).]


2. Describe how you have demonstrated exemplary citizenship in community
(civic, school, or church) related activities.
[Exemplary Citizenship is understanding, appreciating, and doing things that make life better for other people. Citizenship is not just doing the thing that “looks good” or is required. It is doing the thing that will really result in helping others.]


3. Describe how you have demonstrated leadership in school and community activities in which you have participated and the impact you have made.
[Leadership is achieved through inspiring a group of people into higher levels of teamwork and cohesion to accomplish an objective or goal.]


4. Career Goals: What is your occupational goal, why did you choose that profession and what have you done to prepare yourself?


5. Describe how you have exemplified the 4-Way Test in your life?
[The 4-Way Test is a set of questions that exemplify how Rotarians are expected to live. The test asks “Of the things we think, say or do, 1) Is it the truth?, 2) Is it fair to all concerned?, 3) Will it build goodwill and better friendships?, and 4) Will it be beneficial to all concerned?]
